It has been wonderful to be on shift with my mentor Captain, so that I was able to train and have all of my
sign -offs completed.

Description of Activity: Probationary Swim Test Assessment
Comments/Recommendations: On Wednesday, July 29, 2015; I had an opportunity to evaluate Paramedic
Arifa Sikder swimming ability at Pompey Park Pool to assess her progress in meeting the Probationary
Swim Skills Test. As Paramedic Sikder's Mentor Captain, I was disappointed to discover that Paramedic
Sikder has made very little progress in meeting the requirements to successfully pass the Probationary
Swim Skills Test. The Probationary Swim Skills require a firefighter to swim 100 yards without stopping,
swim one length (25 yards) of the pool underwater, tread water for the designated time requirement and
retrieve a 10 -pound weight from 12 feet of water depth.
Paramedic Sikder has agreed to significantly enhance her focus on improving her swim skills at a
location near her residence in Fort Lauderdale. Paramedic Sikder will seek swim skill lessons through
instruction of her choice. We will perform another Swim Skill Evaluation in August 2015 to determine
Paramedic Sikder's progress. A structured Swim Skill remediation program may be required if significant
improvement is not evident at the August Swim Skill evaluation.
This Employee Performance Form is completed for purposes of documentation. Copies of this
form will be delivered to Battalion Chief Mike Twigger, Training Captain Matt Turnbach, Paramedic
Sikder and as documentation for my records.
