TO BE COMPLETED BY EMPLOYEE: Answer the following questions and return this form to your
supervisor.
MAJOR ACCOMPLISIMENT(S): What performance goals have you achieved during this rating
period?
I can proudly say that during this rating period I have successfully completed recruit academy, gained
basic knowledge of street layouts, and have become familiar with apparatus equipment.
EMPLOYEE DEVELOPMENT: What specific training or work assignments have you provided for
other employees to develop their knowledge and shills?
I have been able to use my knowledge from the explorer post that I was involved in to help my fellow
recruits during our training period For example, we had to utilize the chain of command. We, as a
group, elected a class leader and they were the one we directed questions to. If they were unable to assist
with the question, the next step would be to direct the question to our lead instructor. This procedure was
neglected sometimes and the issue was addressed. I explained the importance of the chain of command
and how it is utilized by the fire service on a daily basis.
SUPPORT NEEDED: What type of guidance or other support do you need from your supervisor to
improve your performance during the next rating period?
I look -forward to working with Captain Gibson as my mentor. I am eager to learn more about how DBFR
operates and what is to be expected of me in the field I also am looking forward to Captain Gibson
successfully preparing me for the STEP program that I will be imolved in within the next few months.
SELF-DEVELOPME_VT: What methods/programs/courses, etc. are you using to keep abreast of new
developments in your field?
I am always looking to better myse (as a student of the fire service and have found a few ways to keep up
with the constantly changing field of firefighting and EAE. I learn about new firefighting techniques and
strategies from developed firefighter You Tube pages. I am able to learn different types of hose techniques,
forcible entry techniques, and various other fire ground skills from do'erent men and women around the
country. To keep up with emergency medicine, I work in the emergency room as a paramedic and I am
able to use my IV skills, patient assessment skills, and prehospital experience to be able to better handle
emergency situations in the field.
