MAJOR ACCOMPLISHMENT(S): What performance goals have you achieved during this rating
period?
During this rating period I have successfully completed my 3 month probationary evolutions with help
from senior members of the fire department. I have also successfully completed the first half of my
paramedic S. T.E.P. program and 1 am continuing to improve on my paramedic skills.
EMPLOYEE DEVELOPMENT: What specific training or work assignments have you provided for
other employees to develop their knowledge and skills?
During my S.T.E.P. training, I have involved the rest of my crew to gain their opinions about different
pieces of equipment, patient assessment and emergency driving techniques.
SUPPORT NEEDED: What type of guidance or other support do you need from your supervisor to
improve your performance during the next rating period?
My supervisor has been guiding me very well through my probationary period. I look forward to my
supervisors continued support and guidance throughout the remainder my probationary period.
SELF -DEVELOPMENT: What methods/programs/courses, etc. are you using to keep abreast of new
developments in your field?
I currently hold a position in the Emergency Room as a Paramedic where 1 am able to stay sharp on my
1Y skills, ekg interpretation, and patient assessment. I also participate in the training opportunities the
fire department has to offer such as Target Solutions and hands on training that we do weekly. Each week
I am able to learn something new about the field that I am in which allows me to be a better City of
Delray Beach employee.

I am eager to learn more about the fire service and I believe I can accomplish that with the continued guidance
from my mentor captain, S.T.E.P. trainer and my station crew. I will continue to be the best
