Attachment: PM Flores has fallen behind on his probationary module and, in my opinion, has not demonstrated the initiative expected of a probationary firefighter. He needs to become more assertive in asking station officers or his mentor Captain to complete his probationary modules as well as use of equipment on the apparatus. Whenever training is presented to PM Flores he has a positive attitude, is engaged, participates, and works hard. The shortcoming is when he must start to train on his own. He rarely asks to review any equipment or complete probationary modules. Approximately 3 weeks prior to this evaluation I verbally counseled PM Flores regarding these issues. I followed up with the station officers at his station and reviewed his probationary manual today and have not seen improvement in ` these areas. GA�, G ,,rvi q Te?Sei
