TO BE COMPLETED BY EMPLOYEE: Answer the following questions and return this form to your

MAJOR ACCOMPLISE MENT(S): What performance goals have you achieved during this rating

Completion of DBFR Step program and paramedic sign off. Transitioned from having a step trainer to
being on my own, and feeling confident to make decision and work as a team with other employees.
EMPLOYEE DEVELOPMENT: What specific training or work assignments have you provided for
other employees to develop their knowledge and skills?
Being that 1 and not only a firefighter paramedic but a nurse I am constantly asked question as to why
certain treatments or procedure are done upon arrival to the hospital or what the continued treatments
are to patient. I service as a tool to help bridge the gap to some of their questions. I have also discuss
with several employees the treatment/assessment findings for LVAD patients, which is due to DMC newly
acquired cardiac intervention physicians that are now placing these devices here in our city.
SUPPORT NEEDED: What type of guidance or other support do you need from your supervisor to
improve your performance during the next rating period?
Being that I am a new firefighter paramedic there can be new experiences and learning opportunities
each shift. With each call new and unfamiliar challenges can arise. The shift Captains and crews have
been very helpful in working with me through these situations and I ask for them to, just continue.
SELF -DEVELOPMENT: What methods/programs/courses, etc. are you using to keep abreast of new
developments in your field?
With the implementation of target solutions and assigned learning modules I have been
learning/refreshing materials. I have completed DRI which exposed me to a new aspect of fire rescue and
increased my knowledge of rescue situations and the tools to operate special equipment we have on our
apparatus. I plan attend as many future courses/certifications to become a more rounded rescuer. I am
always reading new material and articles to try and learn about new methods/procedure and the new
challenges that EMSpersonnel are facing each day.
