Hazardous Energy Control Program (RMS-5, Rev. 2)CITY OF DE LR AY B EA C H EM P LOYEE SAFETY MANUAL DEPARTMENT: SUBJECT: REVISION: APPROVED BY: PURPOSE Risk Management Hazardous Energy Control POLICY NUMBER: SUPERSEDES: EFFECTIVE DATE: RMS-5 RMS-5 Rev.1 05/24/2023 Terrence R.Moore,ICMA-CM,City Manager The following Hazardous Energy Control Program,more commonly known as "Lock Out/Tag Out"(LO/TO),has been established to ensure that effective energy (i.e.,electrical,mechanical,etc.)control procedures are maintained to safeguard those employees who operate or perform service or maintenance work on machinery, equipment or processes where the possibility of unexpected energizing,start up or release of any type of stored energy could occur and cause injury.Only electric cord and plug operated machinery and equipment are exempt from the requirements of this Program.However,employees who perform service or maintenance work on this type of machinery and equipment must unplug and maintain the electric plug under their control until all work has been completed.All Departments whose employees perform the above noted activities (except work on electric cord and plug operated machinery and equipment)will participate in this Program.A copy of this written Program is available online on the City's website. POLICY A.Each Department Head is responsible for the administration of this Program as it applies to the Department's various Divisions.This includes the development and maintenance of procedures which are specific to the various Divisions'operations and adhere,at a minimum,to the requirements of this Program. B.Each Department Head will designate a Program Coordinator for the Department or for each Division. The Program Coordinator(s)will then be trained in all aspects of this Program by the City's Safety Coordinator and will subsequently be responsible for ensuring that the requirements of this Program are met.The Program Coordinator(s)will conduct their activities in concurrence with the Department Head, Division Heads and their supervisors,and with the assistance of the City's Safety Coordinator as warranted. C.For the convenience of the Program Coordinator(s),appendices are attached to allow for compliance with the requirements of this Program.Any Department/Division may use other (existing)documentation providing it meets the intent of these appendices. DEFINITIONS •Authorized Employee -An employee who locks out or tags out a machine,equipment,or process to perform service or maintenance work on that machine,equipment or process.The attached Appendix "A"(List of Authorized Lock Out/Tag Out Employees)is a form that the Program Coordinator(s)can use to identify such employees. •Affected Employee -An employee whose job requires him/her to operate a machine or use equipment on which service or maintenance work is to be performed under LO/TO conditions,or whose job requires him/her to work in an area in which such service or maintenance work is to be performed.The attached Appendix "B"(List of Affected Lock Out/Tag Out Employees)is a form that the Program Coordinator(s) can use to identify such employees. •Capable of being locked out -An energy isolating device is considered capable of being locked out if it has a hasp or other means of attachment to which,or through which,a lock can be affixed,or if it has a built-in locking mechanism. •Energized -Connected to an energy source or containing residual or stored energy. •Energy Isolating Device -A mechanical device that physically prevents the transmission or release of energy.Examples include:a manually operated electrical circuit breaker;a disconnect switch;a manually operated switch by which the conductors of a circuit can be disconnected from all ungrounded supply conductors and where no pole can be operated independently;a line valve;a block;and any similar device used to block or isolate energy.Push buttons,selector switches and other control circuit type devices are not energy isolating devices. •Energy Source -Any source of electrical,mechanical,hydraulic,pneumatic,or other energy. •Lock Out -The placement of a lock out device (i.e.,a key lock)on an energy isolating device,in accordance with an established procedure,ensuring that the energy isolating device and the machinery, equipment or process being controlled cannot be operated until the lock out device has been removed by a designated Authorized Employee. •Lock Out Device A device that utilizes a positive means such as a key lock to hold an energy isolating device in a safe position and prevent the energizing of a machine,equipment or process. •OSHA Standard 1910.14 7 -Federal government regulation which can be referenced for hazardous Energy Control information. •Tag Out -The placement of a tag out device on an energy isolating device,in accordance with an established procedure,to indicate that the energy isolating device and the machinery,equipment or process being controlled may not be operated until the tag out device has been removed by a designated Authorized Employee. •Tag Out Device -A prominent warning device such as a tag (and a means of attachment)which can be securely fastened to an energy isolating device,in accordance with an established procedure,to indicate that the energy isolating device and the machinery,equipment or process being controlled may not be operated until the tag out device has been removed by a designated Authorized Employee. PROCEDURE A.As required,all machinery,equipment and processes will be locked out or tagged out to protect against accidental operation during service or maintenance activities. B.A lock out will be used in accordance with an established procedure whenever possible as this is the preferred method to isolate machines,equipment,or processes from energy sources.If an energy isolating device is not capable of being locked out,a tag out will be used in accordance with an established procedure. C.All tags will contain a statement prohibiting unauthorized operation of the energy isolating device and removal of the tag. A.PREPARATION OF LOCK OUT/TAG OUT PROCEDURES Program Coordinators,in conjunction with the applicable Division Heads and their supervisors,will use a Lock Out/Tag Out Procedure Check list (see Appendix "C"attached)to locate and identify all energy sources and energy isolating devices,etc.associated with the machinery,equipment or processes to be locked out or tagged out.Subsequently,based on this information,Program Coordinators will develop:a Machinery/Equipment/Process List (see Appendix "D"attached);specific lock out procedures-see Appendix "E"(Specific Lock Out Procedure)attached;and a specific tag out procedures -see Appendix "F"(Specific Tag Out Procedure)attached.Locks and Tags will conform with and be issued in accordance with Appendix "G' (Locks &Tags)attached. B.LOCK OUT OR TAG OUT ACTIVITY SEQUENCE •The Program Coordinator will provide the Authorized Employee with a written LO or TO procedure for the assigned job. •Whenever this LO or TO procedure is to be implemented,the Authorized Employee will notify all Affected Employees that a LO or TO procedure will be utilized,the reason for it and the location of the machinery, equipment,or process that the procedure will be applied to. •If the machinery equipment or process is operating,the Authorized Employee will shut it down via normal procedures (depress stop button or open toggle switch,etc.) •The Authorized Employee will then operate the energy isolating device(s)so that the machine, equipment,or process is isolated from its energy source(s). •The LO or TO devices will then be applied to the energy isolating devices by the Authorized Employee. The locks and tags will indicate the identity of the Authorized Employee who applied for them.Locks will be installed in a manner that will hold the energy isolating devices in the "safe"or "off'position.Tags, when used,will be affixed to the energy isolating devices in a manner that will clearly indicate that the operation or movement of the energy isolating devices from the "safe"or "off'position is prohibited. •The Authorized Employee will then ensure that all potentially hazardous residual or stored energy is dissipated,disconnected,restrained,or otherwise rendered safe (i.e.,bleeding off pressure in hydraulic or pneumatic power systems until "0"pressure state is attained). •To verify that all energy sources have been disconnected and after ensuring that no employees are exposed,the Authorized Employee will then attempt to activate the operating controls to make certain the machinery,equipment or process will not operate.Operating controls will then be returned to the "safe"or "off'position. •The machinery,equipment or process is now locked out or tagged out. •The Authorized Employee will then remove all guards and safety devices that need to be removed in preparation for service or maintenance work. C.ELECTRICAL WORK Whenever possible,a lock and a tag will be placed on each energy isolating device used to deenergize circuits and equipment on which electrical work is to be performed.If an energy isolating device is not capable of being locked out,a tag will be placed on the energy isolating device without a lock providing the tag out procedure is supplemented by at least one additional safety measure (i.e.removal of an isolating circuit element,blocking of a controlling switch or removal of a valve handle)to reduce the likelihood of inadvertent energization. Once LO and/or TO procedures have been completed,a qualified electrician will be designated to test (utilizing test equipment)circuit elements and electrical component parts of machinery or equipment to which employees will be exposed,before service or maintenance work begins,and to verify that the circuit elements and electrical component parts are deenergized.If the circuits to be tested are over 600 volts,all test equipment must be inspected for proper operation immediately before and immediately after the test activities. D.TESTING/POSITIONING MACHINES OR EQUIPMENT When lock out or tag out devices must be temporarily removed from the energy isolating device(s)and the machine or equipment energized to test or position the machine,equipment,or component thereof,the Authorized Employee will conduct the following activities in the sequence indicated: •Clear the machine or equipment of all tools and materials. •Instruct all Affected Employees to move away from the machine or equipment. •Remove the lock out or tag out devices. •Energize the machine or equipment and cautiously proceed with the testing or positioning activities. •When activities are completed,deenergize all systems and reapply the energy control measures noted in these instructions. D.RESTORING MACHINES/EQUIPMENT/PROCESSES TO NORMAL OPERATIONS When all service or maintenance activities have been completed and the machine,equipment or process is ready to be returned to normal production operations,the Authorized employee will conduct the following activities in the sequence indicated: •Inspect the area around the machine or equipment to ensure that no Affected Employee are exposed. •Remove all tools and materials from the machine or equipment. •Reinstall all guards and safety devices. •Remove all lock out or tag out devices. •Operate the energy isolating devices to restore energy to the machine or equipment. •Advise all Affected Employees that the LO or TO procedure has been completed and that the machine, equipment,or process is ready for normal production operations. E.PROCEDURES INVOLVING MORE THAN ONE EMPLOYEE 1.If more than one Authorized Employee is required to lock out or tag out a machine or equipment,each Authorized Employee will place his/her own assigned lock out or tag out device(s)on the energy isolating devices(s).If an energy isolating device cannot accept multiple locks or tags,multiple lock out or tag out device (hasp)may be used. 2.If a single lock must be used to lock out the machine or equipment,the key for this lock must be placed in a lock out box or cabinet which requires all Authorized Employees to use their own assigned locks to each independently secure the box or cabinet.As each Authorized Employee no longer needs to maintain his/her lock out protection and removes his/her lock,the "last"Authorized Employee can then remove his/her lock from the box or cabinet and subsequently remove the lock out device. F.PERSONNEL OR WORK SHIFT CHANGES In the event of personnel or work shift changes,a reasonable change over period will be established so that the Authorized Employees may exchange their assigned locks and tags.An Authorized Employee who is relinquishing control of an energy isolating device must never remove his/her lock out and/or tag out device until the Authorized Employee who is assuming control of the energy isolating device has properly secured a new lock out and/or tag out device.Authorized Employees who will assume control of the lock out or tag out activities will be thoroughly briefed regarding the scope and stage of current activities by the Authorized Employees who are being relieved. G.REMOVAL OF LOCK OUT/TAG OUT DEVICES BY OTHERS 1.A lock out or tag out device will be removed from each energy isolating device only by the Authorized Employee who applied it,with the following exception: •A lock out or tag out device may be removed by another designated Authorized Employee only if:removal of the lock out or tag out device is critical to operations;and,the Authorized Employee who applied it is not available (i.e.,away from the premises);and,all reasonable efforts have been made to contact this Authorized Employee;and,this Authorized Employee is informed of this situation before he/she resumes work at the facility. 2.To facilitate the removal of lock out devices from energy isolating devices by other designated Authorized Employees,a second set of keys for all lock out locks within the Department will be maintained in a locked box or cabinet under the strict control of the applicable Program Coordinator. H.INFORMING CONTRACTORS Program Coordinators will inform all contractors who may have the occasion to work at the City's facilities of this Program and will ensure that the contractors'programs provide energy controls no less than those outlined in this Program.Program Coordinators will also ensure that both the contractors'and the City's Authorized and Affected Employees are knowledgeable and aware of their respective lock out and tag out responsibilities. I.PROGRAM EVALUATIONS To ensure that this Program is being properly maintained,Program Coordinator(s)will conduct periodic evaluations of this Program,as applied to their respective Departments,documenting activities on the attached Program Evaluation Report (Appendix "H").Any observed deficiencies will be recorded in the report and appropriate corrective actions taken.The report will be maintained as part of this Program until the next evaluation is conducted. J.EMPLOYEE TRAINING 1.All authorized and affected employees will be rained initially regarding the following: •An overview of the requirements contained in this Hazardous Energy Control Program. •Definitions associated with the Program. •The Department's and/or Divisions'procedures for hazardous energy control. •The lock out and tag out devices that will be used. •The sequential activities that are required to properly lock out and tag out machinery/equipment. •Procedures used to verify deenergized electrical circuits. •Activities required to test/position machines/equipment and restore machines/equipment to normal operations. •Procedures involving more than one authorized employee and personnel/work shift changes. •Activities required for the proper removal of lock out and tag out devices. •Program evaluation activities. 2.Program Coordinators will ensure that the above noted training is provided,via verbal instruction and the sue of a provided training video,for all authorized and affected employees.See the attached Employee Training Outline and training Documentation form (Appendix "I"and "J",respectively). Type(s)and magnitude(s)of energy hazard(s): Types(s)and location(s)of energy isolating device(s): Type(s)of stored energy/method(s)to render safe: Name(s)of Authorized Employees: Names(s)of Affected Employees: Appendix "G" CITY OF DELRAY BEACH Hazardous Energy Control Program -Lock Out/Tag Out Locks &Tags Program Coordinators will oversee a formalized lock inventory,issuing key locks only to Authorized Employees for the purpose of locking out machinery,equipment,or processes.Two keys per lock will be provided,one for the Authorized Employee who will perform the lock outs and a second key to be kept in a locked box or cabinet under the strict control of the Program Coordinator.This second key may be issued by the Program Coordinator to another Authorized Employee only under certain conditions-see REMOVAL OF LO/TO DEVICES BY OTHERS elsewhere in this Program.The locks will: be of substantial construction;red in color;and,numbered sequentially (1,2,3-etc.)with lock numbers recorded on Appendix "A"of this Program (identifies which locks are assigned to which Authorized Employees).Care will be taken to ensure that Authorized Employees use only their assigned locks. TAGS Program Coordinators will oversee a formalized tag inventory,issuing tags only to Authorized Employees for the purpose of tagging out machinery,equipment,or processes.The tags will:be substantial enough to prevent inadvertent or accidental removal;be red in color;bear the name of the Authorized Employee who performs the tag outs;be constructed and printed so that exposure to wet conditions or chemicals will not cause the tag to deteriorate or the message on the tag to become illegible;and,include warning messages such as-DANGER,DO NOT START,DO NOT OPEN,DO NOT CLOSE, DO NOT ENERGIZE and DO NOT OPERATE.Care will be taken to ensure that Authorized Employees use only their assigned tags.All tags will be secured to energy isolating devices by a nylon self-locking tie with a minimum unlocking strength of 50 lbs.Removal of the tags will require cutting of the ties. B.Preparation of LO/TO Procedures: i.Explain that the Program Coordinator is responsible for developing all LO/TO procedures in conjunction with the applicable Division Heads and/or their supervisors.And,that this activity requires that the locations and identifies of all machinery,equipment,processes,energy sources and energy isolating devices be determined. C.LO and TO Activity Sequence: i.As described in this Program and the Department's and/or Divisions'procedures,review the activities,in sequence,that are required to properly LO or TO machinery and equipment. D.Electrical Work: i.Review the LO and TO procedures,as indicated in this Program,that must be used when electrical work is to be conducted on machinery or equipment. ii.Review the procedures,as outlined in this Program and the Department's and/or Divisions'procedures, which will be conducted to verify that circuit elements and electrical component parts are deenergized. iii.Note the fact that only a qualified electrical person will be sued for this purpose. Restoring Machines/Equipment to Normal Operations •Ensure that no Affected Employees are exposed to the machine,equipment,or process. •Clear the machine or equipment of all tools and materials. •Reinstall all guards and safety devices. •Remove all LO or TO devices. •Operate the energy isolating devices to restore energy to the machine,equipment,or process. •Notify Affected Employees that the LO or TO procedure has been completed.
